Stuyvesant High School mailed my brother an envelope of forms. My irresponsible younger brother decided to push them aside and fill them out last minute. He misplaced his envelope containing the medical forms and the emergency contact [blue] card. My whole family is now searching for this envelope. -__-

If my brother makes a visit to Stuyvesant High School and asks for another set of forms, will they give it to him? [ó___ò]
