The psychiatrist I saw for over 20 years moved away. How can I find him and get info on my case?

He moved away. I am not sure whether he is still in practice or not. I do not have his CA Medical License number. How can I find out where he is working so I can contact him at his office?

Call the Pharmacy where you had your prescribtions filled, they should have his CA medical license number.

Then call the American Medical Association and ask if they know where he has gone.
