Primary Care Provider Number? (filling out a form)?
Hi, Im filling out a form to get under new insurance at work and Im confused about what they are wanting. I just selected the plan I want and now they want to know the primary care provider #. My Question is this: Do they want my current provider who Im leaving, or do they want me to put a primary care # ONLY if I plan to use them as a secondary provider(which Im not.) My husband is going to use it as a secondary. Here are the titles to the forms to fill in.

Also I dont even know exactly what that number is should it be on my card?. Its not on my medicare card.
Just call your current doctor and ask whoever answers the phone for a NPI and UPIN number, these numbers are how the state can identigy the doctor and make sure they are legit. If the office gives your any trouble just explain why you need it. It’s not a confidential number and they should disclose it to you. NPI number is 10 digits (Ex.: 1851463891 and UPIN number is begins with a letter followed by 5 numbers. (EX.: G80568)
