do I have to give my consent for my employer to contact my doctor again?

I have already givin my consent on 2 seperate occasions, for my employer to speak to my doctor. (same company, different boss.)
I do have a medical condition, but they asked me 2 sign the consent form after i had been off with something completly un-related.
I don’t want to sign this form as I have had to change doctors, because the attitude of my doctor changed towards me after work had hassled her. I don’t want to take the chance of this happening again. Do I have to sign the form? what can happen to me if I don’t?

They may refuse to qualify absenses/payments without ability to verify with the doctor.
