You wanted to leave anyway so your only real concern is if this is a "bad negative record could ruin my pension benefits/insurance and compensation that i recieve as if im not in compliance then i run into problems with the pension board etc."

Your focus should be on making sure that does not happen - and it really shouldn't if all that was at issue was inability to contact you. I would do the following things.
1. Be as nice as possible and try to re-establish a cordial relationship
2. Request a copy of all your records (they must provide those to you) - go and pick them up in person, they may charge a small copying fee
3. Review all notes related to their inability to contact you.
4. If its a simple factual "could not reach" then I'd write them a letter explaining why they couldn't reach you and ask they include it in your record.
5. If there's any negative tone then I'd write them a letter explaining why they couldn't reach you for their record. Then, after enough time for the letter to arrive, call and (nicely) ask them to correct any negative comments with final note in the chart. Request they send/fax you a copy of that too.
6. If any negativity in the notes persists say the inaccuracies may affect your benefits, etc. and you will have to pursue getting it corrected through legal channels...the threat of lawyers should do it as they its A LOT easier to write a "clarifying note" than it is for them to deal with lawyers. Get a lawyer involved if needed.
7. Establish a new physician relationship asap and make sure that goes well and that they have a number they can reach you.

Even all of the above seems like a big mess, I suspect your risk of this really impacting you is low.
References :
Look at it as a blessing because i can not imagine that you could have got any real assistance from a doctor that appears he has much greater issues then yourself.

That said though,if it is going to affect your insurance, benefits or medical records etc negatively then file a complaint against him/them.If your treated this way then other people are too so you owe it to other patients to do something about it.This type of personality shouldnt be a psychiatrist or psychologist as he clearly hasnt dealt with his own issues first so can do much emotional harm to patients.
Doctors are in the service industry just like everyone else that provides a service- a waiter etc- so if this doctor is being rude to patients and the terminating you as a patient is going to affect your records then if they refuse to be reasoned with put in a complaint about this-if you do nothing and the next patient does nothing etc then nothing will ever change and they'll get away with injustices towards people.
References :
Doctors do so much and get so much crap from people just for trying to help them in many cases. Why try to make this guys life harder. Sure maybe he was being a jerk but maybe he had a misfortune day that day. We all can be jerks once in a while. Unless he did something that put someones heath in jeopardy I wouldn't report him, just find another doctor.
Good luck getting better.
References :
you can't do anything to a doctor unless he knowingly and deliberately causes you harm directly. There is no law that says a doctor cannot be an a§§ hole. That is every ones right.
