Does anyone know anything about how to get medical records from Dr. Stephen F. Waller in Conroe, Texas?

I have moved to another state and need to get my medical records from Dr. Waller. Has someone else taken over his practice? who is treating his patients? If anyone has a new phone number or contact info for him it would be greatly appreciated.

I’ve never even been to Texas.. but a Google search shows that his phone number is (936) 539-6771, and his practice is at 333 N Rivershire Drive # 265, Conroe, TX.

I have started a commercial building cleaning service and would like to find a way to get contact info. of general managers for commercial buildings, such as offices,medical ,condo complexes and the like.
I am finding it a diffucult task to get info. about these contacts and am hoping someone may have some ideas for me.

The owners do not handle this sort of thing. You need to contact the property managers they hired to manage the properties.

I searched for "Koty Medical" and I found a device called the "US1000". I then searched for the US1000 and I found this company. The picture looks exactly the same.

i made a dog tag chain thing today at walmart using a machine i put medical issue asthma
and my info and contacts on 2 tags and put the tags on the chain

I’ve seen them. I think it will work, but have considered the following?
1. EMS Star of Life – That way EMT (paramedics) or any bystander will recognize it as your medical alert ID.
2. Make sure to wear it all the time. – Remember that medical ID jewelry speaks for you when you can’t, so you should wear it on your wrist (most preferred place) or neck.

Also, as an added layer of protection, always keep an updated medical ID card in your wallet, which shows not only that you are asthmatic, but what medications are you using.

ARS account resolution contact info?

I live in Florida and have an unpaid medical bill on my credit history the name says "ARS Account Rsolution" I have searched for this company but can not find who it is I am suppose to pay. I have not recieved any letters in the mail so they must not have my correct address. If anyone knows this companies contact info it would be greatly appreciated. Thanks

Request validation with the credit bureaus….they might not be able to validate it…if they can’t, they’ll remove it from your credit report.

Per this law they must validate the item(s) within 30 days or remove them from your credit file. Mail a letter via certified mail with return receipt to all three credit bureaus (Equifax, TransUnion & Experian). Include a photocopy of your driver’s license and social security card…If you don’t include these, they may write back requesting them, which would slow the process down by several weeks. For each negative item, write a separate letter and simply include the phrase:

Personal Info Sent to WRONG Veteran?

Hi I am a Disabled Veteran & I recently requested that the VA mail me a copy of my reward letter to help better explain the breakdown of my disability rating. Anyway instead of receiving the packet that I had requested I receive a few pages with my information + a few pages which clearly showed another veteran’s personal information (first name, last name, spouse name, address, medical info & social). So I contacted the VA right away & they believe that I got this Veterans (let’s call them Veteran X) private paperwork & Veteran X has my private paperwork / personal info. But they do not for sure & at this point there’s really no telling where the VA since my personal information to. Anyway the VA plans on starting an investigation but really I did not need this stress right before the holidays. I plan on calling the DAV in the morning but what else should I or can I do? I cannot believe they made such a big mistake & now my social & home address has been sent to a stranger. Should I file a police report? Do I need a lawyer? Other then wait to see if my social is later misused what can I do?

it may well be that as they have acknowledged that they messed up, do you remember when there was all that info stolen a year or so back, they sent a letter stating that they would pay for monitoring for each person affected.

Get them to agree to this in writing for you, also make copies of this persons info and see if there is any way you can contact him.

I must admit some times I shake my head at the pure incompetence of these people it is NOT rocket science really it is not, right papers in right envelope………….

Sachin, I did use Google and others!

Check around on their site and often you will find a "contact us" link. Sometimes almost buried in that link, at the very bottom of a page with a lot of information, you will find additional contact information.
